Background: Congenital hypothyroidism represents a leading preventable cause of cognitive impairment. Indian data shows an incidence of 1:800 to 1: 1700 new-borns. Objectives: The review outlines the contribution of clinical laboratories in congenital hypothyroidism detection, confirmation and longitudinal monitoring according to current ISPAE and AAP recommendations. Conclusion: Establishing universal cord blood screening with defined alert limits and fast reporting can ensure therapy starts within the first 48 hours.
